Participation in Brazilian Jiu-Jitsu, striking, auxiliary training, camps, seminars, competitions, observation, or related activities at Ares Peoria requires a signed release before you step on the mats. Our waiver covers liability together with photo, video, and promotional media consent—there is no separate media-release flow.
We do not publish a printable PDF. New members complete the online waiver in Wodify(same system we use at the front desk). When you finish, you're added to our CRM as a new lead so we can check you in smoothly.

Use your phone camera on the code below—it opens our Wodify online waiver (identical to the QR on the desk). Complete it beforeyour first time on the mats. If the page doesn't load, ask staff to help you sign in on a tablet at check-in.

Photos, video & promotion
Ares Peoria may capture still photos, recordings, testimonials, signage, newsletters, reels, or live broadcasts that include members, guardians, minors, spectators, seminar guests, and teammates. Details and opt-out expectations align with what you sign in the waiver—coordinate special privacy needs with academy leadership privately before training or larger media events.
